Jim,
You don't have to pay for the first one.  If they didn't update it, then you're not responsible for paying for it.  I would contact the first library and let them know, though.

The library that updated it is the official lender, so they are the ones that get paid.

We just in a book from a fee-charging institution.  However, the institution did not update the transaction to yes/loan.  The request then passed on to the next institution (also fee-charging), which is now sending us a second copy of the book.  How do we avoid paying twice for the same book?
